Evaluation, Monitoring and Treatment of Eye Disease

At Mt Tam Optometric Center, our comprehensive exam is designed to screen for ocular diseases. If we detect evidence of an eye disease, we perform additional testing to provide a specific diagnosis. Finally, we will initiate a plan of treatment (as necessary) by addressing the following goals:

  • Detect and diagnose ocular abnormalities and diseases.
  • Identify risk factors for ocular disease.
  • Identify risk factors for systemic disease based on ocular findings.
  • Establish the presence or absence of ocular signs or symptoms of systemic disease.
  • Determine the refractive and health status of the eye, visual system, and related structures.
  • Discuss the results and implications of the examination with you.

Initiate an appropriate management plan, which may include;

  • further diagnostic tests
  • treatment as indicated, with follow-up visits to monitor progress
  • referral to specialists for further testing or treatment

LASIK Pre and Post Op Care & Free LASIK Evaluation

Dr. Frank has been involved with LASIK since there were only two lasers in the state. He currently does pre- and post-op care for over fifteen different LASIK Centers and is even an “expert witness” in the field. This sort of background makes Mt. Once you have picked a LASIK center, the next step is a pre-op exam. There are several purposes for this exam. The most important is that we want to rule out any physiological reason why you shouldn’t do LASIK. The last thing we want is for you to have a perfect procedure and not see as well as you had hoped. The other purpose has changed over the years. In the early days, our numbers were what drove the laser. Now that most people have Wave Front driven LASIK, our numbers act as a red flag. The surgeons who have worked together with us over the years know that if our numbers are different than those determined by the Wave Front technology, then everything stops until we figure out why.

If all is well, then our staff will help you set up the appointments for the procedure and for your follow up care. Depending on whom you choose to do the LASIK, either the surgeon, or we, will provide the one day follow-up visit. After that we usually see patients at one week, one month, three months, and six months after the procedure. Of course, if additional visits are required, those are included as part of the follow-up care. Most patients are now electing to add RevitalVision® to their Rehab program.

Am I a candidate for LASIK?

Mt Tam Optometric Center, and your LASIK surgeon, will make an evaluation of these factors, and will advise you on the best course for you. The main factors we will examine are:

Refractive Error

LASIKLaserThis corresponds to your glasses prescription. Each laser system has its own range of prescription that it is approved to treat. The LADARVision system can treat from +6.00 diopters of farsightedness to -12.00 diopters of nearsightedness. Also, up to -6.00 diopters of astigmatism can be treated. The limit of treatment in your case will be determined by the thickness and curvature of your cornea before treatment.

Corneal Thickness

We, and your LASIK surgeon will measure the thickness of the clear front surface of your eye (the cornea). This is important because your vision is corrected in LASIK by re-shaping your corneal surface with the laser. This is accomplished by a precise sculpting of the cornea by the laser. The greater your glasses prescription, the greater amount of corneal tissue is removed during the LASIK procedure. Your corneal thickness before LASIK will be measured, and the final thickness after LASIK will be calculated. The LASIK surgeon must be satisfied that the final corneal thickness will be sufficient to avoid complications.

Corneal Curvature

astigmatismYour corneal curvature is part of what determines your glasses prescription. Farsighted people tend to have flatter corneal curvatures, and nearsighted people tend to have steeper corneas. Astigmatism is due partly to unequal vertical and horizontal curves of the corneal surface. Your corneal curvature will be measured, and your final curvature after LASIK will be calculated. Your curvature before and after LASIK must be within a safe range to proceed with LASIK.

Overall Ocular Health

We, and your LASIK surgeon will determine whether there are any underlying conditions that would interfere with obtaining a positive result in your LASIK procedure. Some conditions, such as macular degeneration and cataracts, would affect your level of vision despite a good LASIK outcome. Other conditions, such as central serous retinopathy and prescription fluctuations in diabetes must be resolved before you can proceed with LASIK. Women should wait about 2 months following childbirth to allow their prescription to stabilize after pregnancy.

Cataract Pre and Post Op Care & Free IOL Evaluation

Cataract procedures and what they can do for you have changed a lot in the last few years. Cataract Surgery and Intra-Ocular Lens Replacement

A Cataract Procedure You Can Look Forward To!

The cataract world has changed a lot in the last few years. For many years, the goal of a cataract procedure was to remove the cloudy lens from the eye and then replace it with a man-made single prescription (monofocal) lens. The patient then would rely on glasses or contact lenses to improve their distance or near vision. We at Mt Tam Optometric Center have had the pleasure to work closely with some of the top surgeons of the Bay Area as the new generations of Refractive Implants have become available. This has allowed us to offer more attractive options than ever before to achieve a glasses-free lifestyle.

Refractive Intraocular Lens (IOL) Implants use different technologies to combine both a distance and near vision prescription in a single lens. The multifocal IOL’s such as like the ReSTOR or the TECNIS, act much like bifocal glasses or contacts with both prescriptions in the same lens. The accommodating IOL’s, like the CRYSTALENS, work entirely differently and rely on the muscles inside your eye to change the position of the lens allowing you to change your focus between distance and near.

Plastic Surgery and Pre and Post Op Care

In the last few years Mt Tam Optometric center, has found ourselves doing more and more pre- and post-op care for patients having Blepheroplasty (lid jobs). What is interesting is that we are finding it to be more necessary with the “excellent” surgeons than with the “good” ones. The reason for this is that the “good” surgeons are looking three to four months out when they do the procedure. At his point the initial swelling and bruising is gone so you look great. The problem is that the tissues of the lid continue to relax and four months later you are wondering why the surgeon didn’t tighten it up even more. The “excellent” surgeons tend to look eight months out. The problem with this is that in that eight month period, you are more likely to have dry eye problems because the tissues of the lids have not relaxed enough to allow you to close your eyes completely with most blinks.

It is for this reason that we have started evaluating Blepheroplasty patients for Dry Eye conditions before the procedure. If a patient has dry eye or tendencies towards that, we discuss the options for reducing the dry issues during the rehabilitation period. Then, in conjunction with the surgeon, we lay out a game plan for both pre-op and post-op care.

Special Diagnostic Testing as Requested by Referring Physicians

Mt Tam Optometric Center routinely get requests from physicians for a wide variety of special tests or to follow the progress of a condition or a side effect of a treatment. Some of the more common requests are:

  • retinal changes and potential vision loss related to many of the drugs used for the prevention of a relapse of cancer
  • changes to the retina associated with long standing high blood pressure or diabetes
